If you do not currently have an appointment scheduled, no further visits will be booked. All appointments scheduled on or before March 2 2026 will proceed as planned.
It has been a privilege to support your health over the past 12 years. I am deeply grateful for the trust you have placed in me.
To support continuity of care, I have listed referral options below. You are welcome to choose any practitioner, and I am happy to offer a personal recommendation if helpful. Your choice of provider is entirely your own.
Your clinical records will be securely maintained in accordance with applicable privacy laws. If you would like a copy of your records or wish to have them transferred to another provider, please contact me in writing by email. Written consent is required prior to release. An administrative fee may apply as permitted by law. Records will be retained for the legally required period.
Please note that I will not be available for urgent or emergency care after March 2 2026. For immediate medical needs, contact your primary care provider, visit a walk-in clinic, or attend the nearest emergency department.
